For Adults (18+)

Obtain and complete the name change forms from the Idaho Court Assistance Office website.
Ensure you have all necessary documents filled out correctly.

Submit the completed forms to the Clerk’s Office at your county courthouse. The filing fee is approximately $166.
Keep a copy of the forms and receipt for your records.

Publish a notice of your name change in a local newspaper for four consecutive weeks. Costs range from $70 to $120.
Obtain proof of publication from the newspaper.

Attend the scheduled court hearing. Bring all documents, including proof of publication.
If approved, you will receive a court order confirming your name change.

Ensure both parents or guardians consent to the name change unless one parent has sole legal custody.

I've spoken with a person who works at the Idaho Drivers License office who confirmed this was still valid to use. The confusion seems to be due to the passage of HB421 - the Sex & Gender Bill.
However, I cannot guarantee they will accept this application. If you do find yourself in a circumstance where they aren't accepting the form -
